Clinical Health Coaching
Clinical Health Coaching for Midlife Women
At Awaken Women’s Health, we know that improving your health is rarely about learning more. It’s about implementation.
Most women I work with already know what they “should” be doing. You’ve been told to change your diet, move your body, take your medications consistently, manage stress, sleep more. Your provider may have outlined a clear treatment plan.
But translating that plan into daily life — while managing a career, family, relationships, and the invisible mental load many women carry — is where things break down.
This is where structured support changes outcomes.

Who This Is For
This service is designed for women navigating chronic health conditions who need more than a prescription — they need strategy, accountability, and ongoing clinical guidance. It is particularly helpful for women managing:
- Hypertension
- Type 2 diabetes or insulin resistance
- High cholesterol
- Autoimmune disease
- Gastrointestinal disorders
- Weight-related metabolic conditions
- Perimenopause and menopause-related metabolic changes
If your healthcare provider has created a treatment plan but you are struggling to consistently implement it, this service fills that gap.
What Makes This Different
I am not simply a health coach. I am an experienced medical provider who understands your diagnosis, your lab work, your medications, and how they interact with lifestyle change.
In our work together, I will:
- Review your medical records
- Help you identify meaningful, realistic goals
- Translate complex medical information into practical action
- Support you in managing side effects and barriers
- Help you prioritize what matters most right now
You will not be handed a generic plan. We will build a sustainable framework tailored to your life.
How It Works
Coaching is available in 3, 6, or 12 month packages. Discounts are available for existing patients of Awaken Women’s Health.
Frequently Asked Questions
What does a clinical health coach do?
A clinical health coach helps you bridge the gap between medical recommendations and daily life. Rather than telling you what to do, I help you clarify your goals, develop a realistic strategy, build sustainable habits, navigate the healthcare system, and strengthen communication with your medical team.
Will you prescribe medications?
If you are being treated for a chronic condition, your prescribing provider will continue managing your medications. My role is to support implementation of your existing treatment plan and optimize lifestyle factors that improve outcomes.
Will you give me a diet or workout plan?
We will collaboratively design an eating and movement strategy that aligns with your goals and medical history. If a highly specialized plan is required, I will refer you to the appropriate professional.
Does health coaching actually improve outcomes?
Research supports health coaching as an effective tool for improving outcomes in chronic conditions including diabetes, asthma, and depression. In clinical practice, structured support and accountability can make behavior change easier to sustain.
Who should not enroll?
This service is not a substitute for medical care. If you do not currently have a provider managing your chronic condition, please establish care before beginning coaching. Specialized addiction counseling is more appropriate for active substance use or addiction concerns.
Clinical grounding
Coaching is designed to support behavior change and day-to-day implementation of a clinician-directed care plan.
If you are ready to move from intention to implementation, this is where we begin.
